White Plains, N.Y. (November 22, 2023) – White Plains Hospital has achieved special recognition as an Age-Friendly Health System — Committed to Care Excellence, for showing exemplary efforts to enhance care for all older adults that it serves.
Over the last three months, The Age-Friendly Health Systems Initiative collected data about the number of older adults who received a set of evidence-based elements of high-quality care, known as the 4Ms – What Matters, Medication, Mentation, and Mobility. Proper treatment for an older population includes taking a multidisciplinary approach while establishing and maintaining a plan for continuity of care.
As of August 2023, more than 1,900 hospitals, practices, convenient care clinics, and nursing homes have been recognized as Age-Friendly Health Systems – Committed to Care Excellence. Globally, more than 2.4 million older adults have been reached with 4Ms care.
