Make Your Voice Heard Through the CHNA Survey

For many of us, the communities we live and work in, and that our children play in, are among the most important facets of life.

Equally important is the health of that community: the strength of its health and health services, where needs persist, and the challenges of addressing those needs.

White Plains Hospital, in conjunction with several other local healthcare organizations, is conducting a Community Health Needs Assessment (CHNA) survey – and we need your participation.

Through this confidential and anonymous survey, we are looking to better understand what matters to you and the other members of your community when it comes to your health, as well as the improvements you want to see.

It also is used to establish a general understanding of the state of each participant’s health across their mental, physical, psychological and sexual well-being. Such health assessments can be used not only to further our understanding of your community, but also to enable you to take a proactive stance towards your own health.

Ultimately the results of the CHNA survey – created by the Greater New York Hospital Association – will be used in the development of a plan involving many public partners to improve the health of our community. The results can also impact funding, spending, and other wide-reaching decisions about healthcare.

The 10-minute anonymous survey asks questions about what you think are the most important health issues in our community and what matters to you most. Please follow this link https://www.gnyhasurveys.org/CHNA2022 or the QR code below to complete the survey. The survey closes on June 30th. The survey is available in English and ten other languages. Paper versions of the survey are available by request.

Please share the survey with your colleagues, friends and family and encourage them to take part. All Westchester residents aged 18 and over are eligible to respond to the CHNA.

COVID-19 obviously had a major impact upon all of us, making participation in the CHNA all the more meaningful, so we urge you to participate.

The survey closes on June 30. Make sure your voice is heard!
